banquet reservation form

The Loveland Show Choirs end-of-year banquet will be held at the Oasis Conference Center (902 Loveland-Miamiville Rd.) on Monday, May 12, at 6:30 p.m.  Reservation forms must be completed, paid for (check to Loveland Music Boosters) and turned in no later than Wednesday, April 23. Cost per ticket $25. (No difference in cost between students and family members.)

Show Choir REQUIRED STUDENT Forms

In order to participate in the competition season, Loveland Show Choirs members need to have completed the first two forms.

  1. 2025 Travel Permission Slip - updated 10/15/24 to include South Dearborn and other schedule amendments.

  2. Parent/Student Permission Slip

  3. Permission to Dispense Medication (as necessary but required at beginning of season if so)
